Ohio 21st Century Community Learning Centers grantees must submit their November check-in survey through Microsoft Forms by 11:59 p.m. on Monday, Nov. 9, per a bulletin issued Oct. 30, 2026.
The survey is open now, and the requirement applies to all grantees, the notice says. Unlike some months, this submission does not call for participation trackers to be uploaded alongside the survey.
Grantees with questions or concerns are told to contact OhioAfterSchool@education.ohio.gov.
The bulletin does not describe the survey's questions, estimate how long it takes to complete, or state what happens if a grantee misses the deadline. It also does not indicate whether a confirmation is issued after submission.
